Gastric band: how it works, costs and alternatives

For a long time, the gastric band was one of the best-known operations for severe obesity. How it works, who it is an option for, what it costs and which procedures are chosen more often today.

The essentials in brief

  1. PrincipleAn adjustable silicone band is placed around the upper part of the stomach in keyhole surgery. Above it, a small pouch is created, so that even small amounts can trigger a feeling of fullness.
  2. Adjustable and removableThe band can be tightened or loosened via a small port under the skin. Nothing is removed from the stomach, and the band can be taken out again.
  3. Costs and German health insuranceFor obesity surgery, there is a chance of cost coverage by German health insurance where there is an indication in line with the guidelines, usually from a BMI of 40 or from 35 with related conditions.
  4. AlternativesDepending on BMI and goal, a gastric sleeve, gastric bypass, endoscopic stomach reduction without incisions, a gastric balloon or medically supervised GLP-1 therapy may be an option.

What is a gastric band?

The gastric band – technically “laparoscopic adjustable gastric band”, in English “gastric banding” – is a procedure in obesity surgery, i.e. operations for morbid obesity. A silicone ring is placed around the stomach just below the stomach entrance and constricts it. Above the band, a small pouch is created, known as the pouch. It fills quickly when you eat, and food passes only slowly through the narrowing into the rest of the stomach.

The gastric band works purely restrictively, i.e. by limiting: it restricts the amount that can be eaten at one time. Unlike the gastric bypass, the route food takes through the intestine is not changed, and unlike the gastric sleeve, no stomach tissue is removed.

How the band is adjusted

The band has a chamber on the inside, which is connected via a thin tube to a small port. The port lies under the skin on the abdominal muscles. Through it, a doctor can add or withdraw saline solution with a fine needle; this tightens or loosens the band. Several appointments are usually needed until the right setting is found. Adjustments may also be necessary later, for example if there are complaints or if weight loss stalls.

How does gastric band surgery work?

The band is inserted laparoscopically, i.e. minimally invasively using keyhole surgery and under general anaesthesia. Through a few small openings in the abdomen, the surgical team introduces a camera and instruments, places the band around the upper stomach and secures the port under the skin.

In the first weeks afterwards, the diet is built up step by step, from liquid to puréed to solid food. With the band, the following applies permanently: small bites, chew thoroughly, eat slowly and do not drink at the same time. Bites that are too large or poorly chewed can get stuck in the narrowing and cause a feeling of pressure or vomiting.

Who is a gastric band an option for?

In principle, the same requirements apply to the gastric band as to other obesity operations. The body mass index (BMI) serves as a first point of reference: surgery is usually considered from a BMI of 40, or from 35 if there are related conditions such as type 2 diabetes, high blood pressure or sleep apnoea. In addition, as a rule: conservative attempts to lose weight have been exhausted, the minimum age is 18, there is no pregnancy and none is planned in the immediate future, and the patient’s mental state is stable.

Which procedure is suitable in an individual case also depends on eating habits, related conditions and expectations. Because the band only limits quantity, it has little effect, for example, on high-calorie drinks or soft, sweet foods that pass easily through the narrowing.

Pros and cons of the gastric band

What speaks for the gastric band

  • No tissue removedThe stomach and intestine retain their shape; the procedure is reversible in principle.
  • AdjustableThe narrowing can be adjusted without further surgery.
  • No rerouting of the intestineFood continues to pass through the entire intestine.

What speaks against it

  • Long-term aftercareThe band must be adjusted and checked, often over years.
  • Foreign body in the abdomenThe band, tube and port can slip, leak or cause complaints.

Removing a gastric band: when and how?

A gastric band is removed if it causes complaints or complications, no longer works as intended, or the patient wishes it. Removal is usually also carried out using keyhole surgery. Afterwards, the stomach no longer has a narrowing: the feeling of fullness returns to its previous state, and without further treatment, weight often rises again.

That is why the question of what comes next usually also arises at removal. A switch to another procedure is possible, for example a gastric sleeve or gastric bypass – in one operation or in two steps with a break in between. Which route makes sense depends on the condition of the stomach, on complaints such as reflux and on the course of your weight, and is decided after a gastroscopy and further examinations. Procedures after previous weight-loss surgery are called revision; more on this at Weight Doctors® REVISION.

What does a gastric band cost?

There is no fixed price for a gastric band. The costs depend on the clinic, the scope of preliminary examinations and aftercare, the hospital stay and the anaesthesia. When comparing offers, it is worth looking at what is included: preliminary examinations, surgery, inpatient stay, nutritional support and the adjustments of the band in the months afterwards. Adjustments and check-ups in particular often continue for years with a gastric band and therefore belong in the calculation.

Gastric band and German health insurance

For obesity surgery, there is a chance of cost coverage by the German health insurer where there is a medical indication in line with the guidelines – usually from a BMI of 40 or from 35 with recognised related conditions. This is usually preceded by a 6-month multimodal programme (MMK), with nutritional advice, exercise and psychological support. The MMK is a prerequisite but is not itself a service covered by the health insurer. Without medical necessity, the health insurer does not usually cover the costs. These principles apply to all statutory health insurers, whether AOK, substitute health insurance fund (Ersatzkasse) or company health insurance fund (Betriebskrankenkasse). More on this under Stomach reduction and German health insurance.

Alternatives to the gastric band: what is usually chosen today

Procedures without surgery have been added. A brief overview:

  • Gastric sleeveThe stomach is reduced lengthwise using keyhole surgery, and the larger part is removed. What remains is a narrow tube of 150–200 ml. More on gastric sleeve surgery.
  • Gastric bypassA small pouch is formed from the upper stomach and connected directly to the small intestine. The bypass limits quantity, reduces the absorption of nutrients and changes the metabolism. More on the gastric bypass.
  • Endoscopic stomach reductionDuring a gastroscopy, the stomach is gathered from the inside, without an incision in the abdomen and usually as an outpatient procedure. As with the band, the stomach remains entirely in the body; there is no band that has to be readjusted. More on Weight Doctors SLEEVE®.
  • Gastric balloonA soft balloon partly fills the stomach for 6 or 12 months – a temporary companion, without surgery. More on the gastric balloon.
  • Medically supervised therapy with medicationMedicines modelled on the body’s own satiety hormone GLP-1, usually from a BMI of 30, or from 27 with related conditions. More on Weight Doctors® GLP-1 pharmacotherapy.

Which alternative is suitable depends on BMI, weight-loss goal, related conditions and whether a procedure is an option for you. With a high BMI and pronounced related conditions, surgery is usually the focus; with a lower BMI, a procedure without incisions is more likely.

Frequently asked questions

Questions and answers

Your question isn’t listed? Your specialist will take time for it during the consultation.

Is the gastric band still used today?

Yes, some clinics continue to offer it. Whether a band is suitable in an individual case can be clarified in a consultation at a centre for obesity surgery.

How much weight do you lose with a gastric band?

This varies greatly from person to person. What matters is the adjustment of the band, eating habits, aftercare and physical activity in everyday life. Weight loss is not guaranteed with any procedure.

Can a gastric band be removed without surgery?

No. Removal is a procedure that is usually carried out using keyhole surgery. Without surgery, only the width of the band can be changed via the port.

Does the German statutory health insurer AOK pay for a gastric band?

The same principles apply to the AOK as to all German statutory health insurers: where there is a medical indication in line with the guidelines, there is a chance of cost coverage, usually from a BMI of 40 or from 35 with related conditions, and usually after a 6-month multimodal programme. Without medical necessity, the health insurer does not usually cover the costs.

Which is better: gastric band or gastric sleeve?

A “better” only exists in the individual case. The gastric band is reversible and does not remove any tissue, but needs regular adjustments. With the gastric sleeve, a large part of the stomach is permanently removed; there is no foreign body and no adjustments. Which procedure is suitable depends on BMI, related conditions such as reflux and your expectations.

Can you have another type of weight-loss surgery after a gastric band?

Yes. Switching to a gastric sleeve or gastric bypass is possible, in one or two operations depending on the findings. Beforehand, the condition of the stomach and any symptoms are examined thoroughly.

Can you feel a gastric band in everyday life?

The band itself cannot be felt from the outside, although some people can feel the port under the skin. In everyday life, the band is noticeable mainly when eating: small bites and slow chewing become a habit, otherwise a feeling of pressure or vomiting can occur.
