Treatment paths for dumping syndrome

Gently at first, with targeted correction if needed – always step by step. Which path is an option for you is decided by the diagnostics.

  • Adjusted eating habits

Conservative and always the first step: meal structure, avoiding sugar. Often the symptoms improve with this alone.

  • Endoscopic reduction of the anastomosis

If the connection between the gastric pouch and the small intestine is too wide, it is reduced from the inside so that the pouch empties more slowly – minimally invasive, without another open operation.

  • Minimally invasive revision techniques

Where suitable, we use gentle procedures before a larger procedure becomes necessary.

  • Bypass revision

Surgical adjustment of the anatomy or the loop lengths in order to slow emptying.

Dumping syndrome after bypass at a glance

Where the symptoms come from, how we determine the form and which path makes eating easier for you again.

Early and late dumping: what happens after eating

In a gastric bypass, a small stomach pouch is connected directly to the small intestine; food therefore reaches the intestine more quickly than before. In some patients, this can cause dumping syndrome: circulatory and digestive complaints that affect eating and thus everyday life. It is a known accompanying effect and can be treated.

Whether early, late or mixed dumping is present determines the right treatment – that is why we first classify the form precisely:

Hands of the surgical team guiding the instruments through small access points in the abdominal wall
  • Early dumpingShortly after eating, triggered by emptying into the small intestine too quickly.
  • Late dumping1–3 hours after eating, triggered by reactive low blood sugar.
  • Mixed formsMany of those affected have elements of both. Even then, the treatment can be tailored specifically to this.
  • Sensitive sugar processingIn addition to the rapid passage through the stomach, sugar processing plays a role – after the bypass, the body reacts more sensitively to sugar and fast carbohydrates. This can be compensated for in a targeted way.

Typical symptoms of dumping syndrome

These complaints after a gastric bypass can point to dumping syndrome. You will find everything about the specialist clinic, further complaints after stomach surgery and aftercare under Weight Doctors® REVISION.

Weight Doctors® staff member on the telephone at initial contact
  • Early dumping (shortly after eating): nausea, racing heart, sweating, dizziness, abdominal cramps, diarrhoea
  • Late dumping (1–3 hours afterwards): low blood sugar, trembling, cravings, weakness, difficulty concentrating
  • Weak spells up to circulatory collapse
  • Complaints above all after sweet or carbohydrate-rich meals
  • Fear of eating – meals and social situations are avoided

“Nobody should avoid eating out of fear of the next weak spell. With dumping syndrome, there are clear causes – and effective ways to resolve them.”

Our approach – step by step

Clearly determining the form and cause – this is the basis of every treatment:

Doctor guiding the endoscope during an examination
  1. Initial contactIt begins with a detailed conversation – in person, by telephone or by video consultation.
  2. Detailed analysisWith a dumping score, we record your symptoms, your diet and your history in a structured way.
  3. Targeted diagnosticsA glucose tolerance test shows how your body reacts to sugar. An endoscopy shows how wide the connection between the gastric pouch and the small intestine (anastomosis) is.
  4. Clear identification of the causeFrom all of this together, we classify whether early, late or mixed dumping is present – and which path is the right one.
  5. Individual treatment strategyYour plan – from diet to correction of the anastomosis.

Gently at first: adjusting eating habits

Conservative therapy is always the first step – and often the symptoms already improve considerably with it. The focus is on a fixed meal structure and consciously avoiding sugar and fast carbohydrates, which with a bypass trigger symptoms particularly easily.

Our nutrition professionals support you with this in person and digitally, for example in Weight Doctors® 360° nutrition coaching. If the change is not enough, that is not a failure: there is then often an anatomical cause that can be corrected.

Two Weight Doctors® staff members with fresh fruit and vegetables

Endoscopically from the inside: reducing the connection

If the anastomosis is too wide, the gastric pouch empties too quickly. The connection can then be reduced again endoscopically – that is, through the mouth, from the inside – so that the pouch empties more slowly. This is minimally invasive and requires no further open operation.

Where it is suitable, we use such gentle revision techniques before a larger procedure becomes necessary. Which technique suits you is decided by the findings of the endoscopy.

Bypass revision: when the anatomy needs to be adjusted

If the dumping cannot be sufficiently relieved with diet and endoscopic means, a surgical bypass revision may be an option. The anatomy is adjusted, for example the length of the small intestine loops, in order to slow emptying.

Surgical procedures take place at the Weight Doctors® Nationalklinik in Wertheim, carried out by specialists in obesity surgery and corrective procedures – surgical and endoscopic techniques under one roof.

Frequently asked questions

Questions about Weight Doctors® REVISION dumping syndrome

Your question isn’t listed? Your specialist will take time for it during the consultation.

What is dumping syndrome?

A known accompanying effect after a gastric bypass: because food reaches the small intestine more quickly, circulatory and digestive complaints can occur after eating – from nausea and a racing heart to low blood sugar. It occurs in only some patients and can be treated.

What is the difference between early and late dumping?

Early dumping is caused by emptying into the small intestine too quickly and shows shortly after eating. Late dumping is based on reactive low blood sugar and comes 1–3 hours later. Many people have mixed forms.

After which meals do the symptoms occur?

Above all after sweet or carbohydrate-rich meals. After the bypass, the body reacts more sensitively to sugar and fast carbohydrates.

Is dumping syndrome dangerous?

The symptoms can range up to weak spells and circulatory collapse, and many people avoid meals or social situations out of fear. Have recurring symptoms checked rather than putting up with them.

Does dumping syndrome go away?

In many people it improves considerably with a targeted change in diet. If that is not enough, the anatomical causes – for example an anastomosis that is too wide – can be corrected endoscopically or surgically.

Can dumping be treated without an operation?

Often yes. The first step is always conservative therapy with a fixed meal structure and little sugar. If that does not help sufficiently, a gentle endoscopic reduction of the connection is often possible – without an open operation.

How is the anastomosis reduced?

If the connection between the gastric pouch and the small intestine is too wide, it is made narrower again from the inside, that is, endoscopically. This slows the emptying of the pouch and eases the dumping.

How is dumping syndrome diagnosed?

We record your symptoms, your diet and your history with a dumping score. In addition there are examinations such as a glucose tolerance test and an endoscopy, which shows the size of the anastomosis.

I was operated on at another clinic. Can I still come?

Yes, especially then. Our revision clinic is open to everyone – regardless of where your bypass was created.

Does the German health insurance cover the treatment?

In proven dumping that does not respond to therapy, and where the indication is in line with the guidelines, there is a chance of cost coverage. There is no prior application: before the procedure, we check all the requirements for you, and you do not have to speak to your German health insurer.

Can I pay for the treatment in instalments as a self-paying patient?

Yes, part payment is possible via our partner medipay. Prices for Weight Doctors® REVISION start at €900; more under Payment in instalments.

Where does the treatment take place?

The consultation is possible in person, by telephone or by video. Procedures take place at the Weight Doctors® Nationalklinik in Wertheim.
