Weight loss five years after the endo sleeve what a long-term study shows

One of the longest studies on the endo sleeve with OverStitch™ followed the weight loss of 216 patients over five years. An overview of the design and results.

As of today: The post was published in 2022. The study examined the OverStitch™ technique of the time; at Weight Doctors®, OverStitch NXT™ or pose® 3.0 Double Helix are used today.

The procedure

What is the endo sleeve with OverStitch™?

The endo sleeve with the nickel-free OverStitch™ technology – in technical terms endoscopic sleeve gastroplasty – reduces the stomach permanently and comes into question for people with severe excess weight and obesity. Under general anaesthesia, an endoscopic suturing device is carefully guided through the mouth and oesophagus into the stomach; anchored sutures (plications) gather the stomach from the inside.

The procedure is intended to achieve a result similar to a surgical sleeve gastrectomy (gastric sleeve operation), without the need for an operation. By comparison, it is considered lower in risk, recovery is shorter, and no incisions or scars remain.

The endo sleeve is suitable above all for people with a BMI of 30 or more who have already tried other ways of losing weight – such as diet and exercise – without lasting success.

The study

One of the longest studies on the endo sleeve

Most studies on weight loss in excess weight and obesity run for one to two years. The work by Sharaiha and colleagues (2021), by contrast, is among the longest studies on the endo sleeve with OverStitch™: it followed weight loss for up to five years after the procedure.

Patients were included in the study if they met one of the following conditions:

  • a BMI over 30, or over 27 with related conditions, if other, non-invasive methods had previously achieved no more than 5% weight loss
  • a BMI over 40, if bariatric surgery was declined or weight had to be lost before an operation
  • previous medication to lose weight, after which weight stayed stable for three months or during which weight was gained

The average age was 46 years (33–59 years), and the average BMI before the procedure was 39 (33–45). 213 of the 216 patients had not previously had a bariatric procedure.

The aim

What was examined

The main aims were the loss of total body weight (total body weight loss, TBWL) and the loss of excess weight (excess weight loss, EWL) after the endo sleeve. Weight was measured before the procedure and at routine check-ups after 1, 3, 6, 12 and 24 months, and annually thereafter.

In addition, accompanying medication was offered if a weight loss of at least 5% was not achieved in the first three months after the procedure, or if weight rose again at any point afterwards.

The results

Weight loss after 1, 3 and 5 years

Based on starting weight and excess weight, the study evaluated TBWL and EWL at the check-ups after 1, 3 and 5 years:

TimingTotal body weight (TBWL)Excess weight (EWL)
after 1 yearon average 15.6%; the great majority lost 5–10% of their body weighton average 47.9%; 80% achieved an EWL of at least 25%
after 3 yearson average 14.9%; 85% maintained a weight loss of 5%, 63% of 10%on average 45.1%; 68% retained an EWL of at least 25%
after 5 yearson average 15.9%; 90% maintained a TBWL of 5%, 61% of 10%on average 45.3%; 74% retained an EWL of at least 25%

75% of patients did not regain weight over the course of the study. 27% had a slight weight gain between the 3rd and 9th month after the procedure and therefore began medication – it helped to prevent further gain, but weight loss was not greater as a result compared with the patients without medication.

Conclusion

What the results mean

The study is one of the few to have examined weight loss after the endo sleeve with OverStitch™ over five years. After five years, 90% of patients had a TBWL of 5% or more and 74% an EWL of 25% or more – an indication that the endo sleeve can be an effective procedure for the treatment of obesity. Results vary from person to person.

This is significant because even a weight loss of 5% can noticeably improve health and reduce the risk of related conditions such as high cholesterol levels, type 2 diabetes and cardiovascular disease. The results are promising; further long-term studies are needed to understand the endo sleeve even better as a long-term treatment.
