Losing weight with a doctor: when medical help makes sense

Losing weight with a doctor is not a sign of weakness but, in the case of obesity, the medically recommended approach. Obesity is a chronic disease involving metabolism, hormones, predisposition and life circumstances. Medical support helps to identify causes, treat related conditions and find an approach that suits your own situation.

Medical support makes particular sense if:

  • your BMI is 30 or aboveFrom this value, it is referred to as obesity. You can calculate your value with the BMI calculator.
  • you have related conditionssuch as high blood pressure, type 2 diabetes, raised blood lipids, fatty liver, joint problems or sleep apnoea – in which case even with a lower BMI.
  • several serious attempts have not helpedIf your weight does not go down despite changing your diet, or keeps coming back.
  • your weight is a burden to youphysically, in everyday life or emotionally.

The guide I want to lose weight but can’t manage it explains which medical reasons can lie behind unsuccessful attempts to lose weight.

The GP practice: the first port of call

For most people, the journey begins at the GP practice. Your GP knows your history and your medication and can arrange the most important examinations:

  • MeasuringWeight, height, BMI, waist circumference and blood pressure.
  • Blood valuesBlood sugar and long-term blood sugar (HbA1c), blood lipids, liver values and the thyroid value TSH.
  • Checking causessuch as medication that increases weight, or signs of sleep apnoea.
  • ReferringReferral to specialists and, if necessary, a medical certificate for nutritional counselling.

Where there is a medical reason, this assessment is covered by German statutory health insurance. However, the GP practice often lacks the time for longer-term weight-loss support. That is why GPs frequently work together with nutrition professionals and specialised facilities.

Nutritional medicine physicians: what the title means

A nutritional medicine physician is a doctor, usually with specialist training in general or internal medicine, for example, who has additionally completed further training in nutritional medicine. They can assess nutrition and diseases together, for example in the case of diabetes, fatty liver or after weight-loss surgery, and initiate medical therapy.

There are also non-medical nutrition professionals. The titles say different amounts about their training:

Dietitians: a state-recognised health profession with a protected professional title.

Oecotrophologists and nutrition scientists: a university degree in nutrition and home economics.

Nutrition advisers: not a protected title – the qualification can range from a university degree to a short course.

So ask about their training. Many German health insurers provide a subsidy for nutritional counselling with a medical certificate if the counselling is carried out by a recognised professional. It is best to clarify this with your health insurer in advance.

Specialists for specific questions

Depending on the findings, the GP practice refers you to the appropriate specialty:

Internal medicine, endocrinology and diabetology

In the case of abnormal blood sugar levels, diabetes, thyroid disease or other hormonal disorders. Specialists in internal medicine also assess the heart, circulation and metabolism as a whole and can supervise drug-based obesity therapy.

Gastroenterology

In the case of raised liver values, fatty liver, heartburn or stomach problems. Gastroenterologists carry out gastroscopies – the basis for endoscopic procedures such as the gastric balloon or stomach reduction without an incision.

General and visceral surgery

If weight-loss surgery such as a gastric sleeve or gastric bypass is an option. Operations are performed in hospitals with experience in obesity surgery, usually after structured preparation.

Gynaecology

In the case of menstrual cycle disorders, suspected PCO syndrome or an unfulfilled wish to have children. This hormonal disorder is frequently linked to excess weight and insulin resistance.

Psychotherapy and psychiatry

In the case of binge eating, emotional eating, depression or severe emotional strain caused by weight. Psychological support is also an integral part of preparing for weight-loss surgery.

Professional help with weight loss: what to look out for

Alongside doctors’ practices and hospitals, there are many services that promise professional help with weight loss: programmes, coaches, apps and “obesity advisers”. This title is not protected; it may stand for medical, nutritional science or entirely different qualifications. You can recognise reputable services by these points:

  • Medical involvementPre-existing conditions and medication are asked about, and a doctor is involved if needed.
  • Transparent qualificationsThe training and names of the advisers are openly stated.
  • Realistic goalsno promises of a certain number of kilos in a short time.
  • Transparent costsPrices, duration and cancellation terms are clear in advance.
  • Support beyond weight lossMaintaining your weight is part of the plan.

Medical support by video can also make sense, for example for follow-up checks. A prerequisite is a thorough medical history taken by a specialist – especially if medication is to be prescribed.

Which doctor suits your situation?

For guidance, without replacing the medical assessment in the individual case:

  • Slight excess weight without symptomsGP practice for assessment, plus nutritional counselling from a recognised professional.
  • Obesity from a BMI of 30 or excess weight with related conditions: GP practice, nutritional medicine or internal medicine; medically led therapy without a procedure may be an option.
  • Abnormal hormone, sugar or liver valuesSpecialist in endocrinology and diabetology or in gastroenterology.
  • High BMI or several unsuccessful therapiesan obesity centre that also covers endoscopic procedures and weight-loss surgery.
  • Binge eating or severe emotional strainpsychotherapy in addition.

What does German health insurance pay for?

  • Diagnosis and treatment of related conditionscovered by German statutory health insurance where there is a medical reason.
  • Nutritional advicewith many German health insurers, a subsidy with a medical certificate.
  • Medically led therapy with weight-loss medication: currently not covered by German statutory health insurers; private insurers cover the medical services depending on the contract.
  • Weight-loss surgeryWhere there is an indication in line with the guidelines – often from a BMI of 40 or from a BMI of 35 with related conditions – there is a chance of cost coverage by German health insurance. The multimodal programme for preparation is not covered by statutory health insurance. Details under Stomach reduction and health insurance.

Frequently asked questions

Questions and answers

Your question isn’t listed? Your specialist will take time for it during the consultation.

Which doctor helps with weight loss?

The first port of call is the GP practice. It investigates causes and related conditions and refers you on if needed – to nutritional medicine, internal medicine, endocrinology, gastroenterology or psychotherapy. In the case of severe excess weight, an obesity centre in which these specialties work together makes sense.

Can my GP help me lose weight?

Yes. Your GP can check blood values, identify causes such as medication or an underactive thyroid and treat related conditions. For longer-term support, the practice often works together with nutrition professionals or an obesity centre.

What does a nutritional medicine physician do?

A nutritional medicine physician is a doctor with an additional qualification in nutritional medicine. They assess nutrition and diseases together, for example in the case of diabetes or fatty liver, and can initiate medical therapy. Nutritional counselling without medical training is no substitute for this.

What is an obesity adviser?

The title is not protected. It may stand for nutrition professionals, coaches or doctors. Ask about their training and whether a doctor is involved in medical questions.

Does German health insurance pay for a doctor to help with weight loss?

German health insurers cover the medical examination and the treatment of related conditions where there is a medical reason. Many health insurers provide a subsidy for nutritional counselling. Statutory health insurers currently do not pay for weight-loss medication and many weight-loss programmes.

Do I need a referral to the obesity centre?

That depends on the centre and on the planned approach. A referral is often not needed for an initial consultation. For an operation funded by German statutory health insurance, medical documents and documented preparation are required.
