GLP-1 explained honestly: effects, limits and what can come next

Why GLP-1 pharmacotherapy helps many people but is not the whole answer for everyone – and which paths there are when the injection alone is not enough.

Between turning point and dead end

Rarely has a treatment been as controversial as the GLP-1-based weight-loss injection. For some it is a turning point, for others a medical dead end. As so often, the truth lies somewhere in between.

At Weight Doctors®, we talk every day with patients who have started GLP-1 pharmacotherapy – some euphoric, some frustrated, some simply at a loss. This article aims to bring clarity: what GLP-1 can do, where it reaches its limits, and how you can find the next step without the detour of trial and error.

The effects

What GLP-1 is – and why it excites so many people

GLP-1 (glucagon-like peptide-1) is a hormone produced by the gut that is released after eating and regulates the feeling of fullness. Modern medications contain active substances that imitate this hormone or strengthen its effect – in everyday language, this is called “GLP-1 pharmacotherapy”.

The effects of GLP-1 therapy: the stomach empties more slowly, the feeling of hunger is dampened, and cravings ease. Clinical studies show an average weight loss of 15 to 22.5% of body weight – more than many conventional methods achieve.

For many people worldwide, GLP-1 therapy is therefore a real turning point. Besides weight, related conditions such as type 2 diabetes, high blood pressure or lipid metabolism disorders can also improve. Anyone who starts GLP-1 pharmacotherapy is doing something for their health – that is also part of the truth.

15 to 22% sounds like a lot – what does it really mean?

At first glance, the study results look impressive. Behind the percentages, however, is a reality that often gets lost in the public debate – and that is crucial for you.

In practice, a weight loss of 15 to 22.5% means a reduction of around 10 to 15 kilograms for most patients – rarely more. With moderate excess weight, that may be exactly the goal. With pronounced obesity and a BMI well above 35 or 40, it is a good start, but rarely the end of the journey.

In other words: GLP-1 pharmacotherapy is a powerful tool, but not a miracle cure. It brings many people to a point where they realise: there is still some way to go.

Results vary from person to person.

The limits

The other side – where GLP-1 reaches its limits

However convincing the evidence on effectiveness is, everyday life often looks different. Dr. med. Roland Mörmel, specialist in internal medicine at Weight Doctors®, puts the international data into context:

“Up to 65% of patients stop the therapy within two years. Side effects such as nausea, vomiting and diarrhoea are common. After stopping, between two thirds and over 80% of the lost weight comes back.”

Dr. med. Roland Mörmel, specialist in internal medicine

Why many people do not reach their goal with GLP-1 alone

  • A plateau on the scalesAfter good initial success, weight stops falling despite continued therapy.
  • Side effectsNausea, vomiting and diarrhoea noticeably affect everyday life.
  • CostsThe monthly costs add up; statutory health insurers in Germany do not pay for the therapy for weight reduction.
  • The question of duration“Do I want this for the rest of my life?” – many people ask themselves this at some point.

That does not mean GLP-1 is bad. It means that with the right concept behind it, GLP-1 pharmacotherapy can develop its full effect – without that concept, it remains an interim solution for many.

Bridge therapy

GLP-1 is not the end,
but often a building block

GLP-1, gastric balloon and endo sleeve are not mutually exclusive. On the contrary: used wisely, they complement each other.

We call this bridge therapy – a well-thought-out step-by-step concept in which each treatment works where it is strongest. This is exactly what distinguishes a medical strategy from a mere product recommendation. The transition is supported by doctors and adapted to your progress.

  1. Gastric balloon → GLP-1You lose weight with the gastric balloon over 6 or 12 months and then use GLP-1 pharmacotherapy to stabilise the weight you have reached.
  2. Endo sleeve → GLP-1After permanent stomach reduction with pose® 3.0 Double Helix or OverStitch NXT™, GLP-1 therapy can help as a supplement during the stabilisation phase.
  3. GLP-1 → Endo sleeveIf the weight-loss injection reaches its limits, a one-off, long-term treatment takes over – entirely without conventional surgery.

At Weight Doctors®, we do not plan individual treatments, but your whole journey.

Weight Doctors SLEEVE®

The endo sleeve – if you are looking for a lasting solution

If you do not reach your goal with GLP-1 pharmacotherapy – or want a one-off, lasting solution from the start –, you will find an alternative to conventional stomach reduction in Weight Doctors SLEEVE®.

With the endoscopic procedures pose® 3.0 Double Helix or OverStitch NXT™, the stomach volume is reduced by up to 70–75%. The procedure is minimally invasive, performed through the mouth – without external incisions, without scars, as an outpatient – and is designed to be long-term.

What does that mean in everyday life? You feel full sooner and naturally eat less – without a weekly injection. A one-off treatment instead of a monthly therapy. And where there is a medical indication in line with the guidelines, there is a chance of cost coverage by your German health insurer.

Four myths about GLP-1 – and what is true

  • “If I stop, the weight stays off”Wrong. In two thirds of people, a large part of the lost kilos comes back after stopping. This is not a sign of a lack of discipline, but a biological reaction of the body.
  • “GLP-1 replaces a procedure”No. GLP-1 pharmacotherapy can prepare for, accompany or complement a procedure. For some people, however, permanent stomach reduction is the more honest and more sustainable path.
  • “Online pharmacies are as good as medical support”This is not just a myth, but a risk. GLP-1 therapy belongs in the hands of a specialist – with individual dosing, regular check-ups and care that continues even after stopping.
  • “GLP-1 is a fad”That is not true either. GLP-1 is well researched scientifically. The question is not whether it works, but for whom and for how long.

Frequently asked questions

Questions and answers

Your question isn’t listed? Your specialist will take time for it during the consultation.

Who is GLP-1 suitable for?

GLP-1 therapy can be a sensible option if you have obesity, prefer a medication-based path without a procedure and would like to be supported by doctors. Realistic expectations are important: GLP-1 pharmacotherapy helps with losing weight, but does not replace a change in lifestyle. Whether it suits you – or whether another path makes more sense – we clarify in the no-obligation consultation.

How much weight can I really lose with GLP-1?

In clinical studies, the average weight loss was 15–22.5% of body weight; in everyday life, that is around 10–15 kg for most people. With moderate excess weight, that may be enough; with pronounced obesity, it is often a good start, but rarely the end of the journey. Results vary from person to person.

Does German health insurance cover the cost of GLP-1?

In Germany, statutory health insurers currently do not pay for GLP-1 medications for weight reduction alone; the therapy is a self-pay service. It is different with weight-loss surgery or the endo sleeve: here, there is a chance of cost coverage where there is a medical indication in line with the guidelines. There is no advance application: billing takes place directly after the procedure.

What happens if I stop the weight-loss injection?

In two thirds of people, a large part of the lost weight returns after stopping – a biological reaction of the body, not a personal failure. For that reason, do not stop the therapy without consulting your doctor, and talk early about follow-on options such as bridge therapy or an endo sleeve.

What is bridge therapy and how does it work?

A step-by-step concept in which GLP-1, gastric balloon and endo sleeve complement each other instead of competing. For example, you lose weight with the gastric balloon and then maintain the weight with GLP-1 therapy – or switch to Weight Doctors SLEEVE® if the injection reaches its limits. Our doctors support the transition.

What distinguishes the endo sleeve from conventional stomach reduction?

With the endo sleeve (Weight Doctors SLEEVE®), the stomach is reduced endoscopically through the mouth – without external incisions and scars. With conventional stomach reduction (gastric sleeve or gastric bypass), surgery is performed laparoscopically, with small incisions in the abdominal wall. Both procedures have their place; which one suits you depends on BMI, related conditions and your goals.

Is the consultation really no obligation?

Yes, completely. We advise you on all paths – GLP-1, gastric balloon, endo sleeve, weight-loss surgery and RESHAPE – and recommend what suits your situation. Only when you decide on a path do we move forward.
