Does the German health insurer pay for liposuction?

Do you suffer from lipoedema and wonder whether your German health insurer will cover the cost of liposuction? Under certain conditions, cost coverage is possible – with both statutory and private health insurance.

Here you can read exactly which conditions apply, which doctors may make the diagnosis and provide the expert report, and how Weight Doctors® RESHAPE supports you on the way to treatment.

Private health insurance (PKV)

Lipoedema treatment with private health insurance

Whether private health insurance pays depends on your policy and on any exclusions of cover. If lipoedema is included in your policy’s benefits, the following evidence is needed for the application – we take care of the application and all documents completely:

  1. Specialist diagnosisThe initial diagnosis must be made by a specialist in internal medicine and angiology, in physical and rehabilitation medicine or in dermatology – or by a specialist with additional training in phlebology.
  2. Conservative pre-treatmentMedically supervised therapy lasting at least six months, documented without gaps, is required.
  3. Stable body weightWith monthly measurements at your GP’s practice, you demonstrate that your weight has remained stable over the documentation period.
  4. Expert report on the operationThe certificate of the medical necessity of liposuction is issued by a specialist in plastic, reconstructive and aesthetic surgery, by a specialist in another surgical field who performs operations, or by a dermatologist.

What we prepare for your private health insurer

  • We go through the exact policy terms with you.
  • We put together the clinic’s cost estimate and the evidence of therapy for your insurer.
  • We clarify whether there are special lipoedema programmes – some insurers, for example HanseMerkur, offer such programmes.
German statutory health insurance (GKV)

Lipoedema surgery through the German statutory health insurance (GKV): the conditions in detail

The German statutory health insurer covers the cost of lipoedema surgery under clearly defined conditions. First, lipoedema must be confirmed. For this, the following features must be present:

  • Symmetrical increase in fatty tissue on the arms and/or legs
  • Hands and feet are not affected and remain slim
  • Pain on pressure and touch in the soft tissue of the affected body regions

Who may make the diagnosis for the German health insurer?

The diagnosis may be made exclusively by specialists in these fields:

  • Internal medicine and angiology
  • Physical and rehabilitation medicine
  • Dermatology
  • Additional training in phlebology

Important to know: GPs or plastic surgeons – even if they are lipoedema specialists who perform operations – may not make the initial diagnosis within the meaning of the decision of the Federal Joint Committee (G-BA).

BMI limits and further conditions

For the German statutory health insurance (GKV) to approve the operation, all of the following points must be met:

  • Conservative therapy has been prescribed by a doctor, carried out continuously for 6 months and has not relieved the symptoms sufficiently.
  • You have not gained weight in the last 6 months (documented monthly at the GP’s practice).
  • Psychological factors have been recorded and taken into account.
32–35 kg/m²
Liposuction is permitted only if the excess weight is predominantly caused by the lipoedema (observe the limits of the waist-to-height ratio, WHtR)
from 35 kg/m²
Treatment of obesity is required first
from 40 kg/m²
Liposuction through the German statutory health insurance (GKV) is not possible

Who may perform lipoedema surgery?

Lipoedema surgery may be performed by specialists in plastic, reconstructive and aesthetic surgery, by specialists in other surgical fields who perform operations, and by dermatologists.

For January 2026, the Association of Statutory Health Insurance Physicians announced that it would publish an official list of approved doctors and clinics.

What does the German statutory health insurance (GKV) cover – and what does it not?

  1. The German statutory health insurance (GKV) pays for thisLiposuction of the arms, legs and, where applicable, the buttocks (depending on findings), as well as anaesthesia, inpatient monitoring and the usual aftercare.
  2. The German statutory health insurance (GKV) does not pay for thisLiposuction of the abdomen, back or chest, and skin tightening after liposuction.

Good to know: for lipoedema at stage I and II, treatment is currently carried out only as an outpatient, with a maximum aspiration volume of three litres.

Frequently asked questions

Questions and answers

Your question isn’t listed? Your specialist will take time for it during the consultation.

Does the German statutory health insurer cover the cost of lipoedema surgery?

Yes, under certain conditions: a diagnosis by an approved specialist field, documented conservative therapy lasting at least 6 months without sufficient improvement, a stable weight and compliance with the BMI limits. From a BMI of 40 kg/m², liposuction through the German statutory health insurance (GKV) is not possible.

What does the German statutory health insurer pay for in lipoedema surgery – and what not?

Covered are liposuction of the arms, legs and, depending on findings, the buttocks, plus anaesthesia, inpatient monitoring and the usual aftercare. Not covered are liposuction of the abdomen, back or chest and skin tightening after liposuction.

Which doctors may make the lipoedema diagnosis for the German health insurer?

According to the decision of the Federal Joint Committee (G-BA), only specialists in internal medicine and angiology, in physical and rehabilitation medicine or in dermatology, as well as doctors with additional training in phlebology. GPs and plastic surgeons may not make the initial diagnosis.

Which BMI limits apply to cost coverage by German health insurance of lipoedema surgery?

With a BMI of 32–35 kg/m², liposuction is permitted only if the excess weight is predominantly caused by the lipoedema. From a BMI of 35 kg/m², obesity must be treated first; from 40 kg/m², the German statutory health insurer does not pay for lipoedema surgery.

Does private health insurance cover lipoedema surgery – and what does it need for the application?

Whether private health insurance pays depends on your policy. What is needed is the diagnosis by an approved specialist field, documentation of the conservative therapy (at least 6 months), evidence of a stable weight, a medical certificate of medical necessity and a cost estimate from the clinic. We put together the application and all documents for you.

Can I combine lipoedema surgery with obesity treatment?

Yes, at Weight Doctors® you get both from a single source. If your BMI is 35 kg/m² or higher, you first reduce your weight – for example with Weight Doctors BARIATRICS®, the Weight Doctors SLEEVE® or Weight Doctors® GLP-1. The lipoedema surgery then follows as part of Weight Doctors® RESHAPE.
